Chronic urinary retention may cause few or no symptoms. A painless bladder scan shows what you leave behind.

Cut From Steel

KNOCKED DOWN IN ROUND ONE. HE FINISHED IN THIRTEEN.

Rocky Marciano grew up in Brockton, Massachusetts, and wanted to be a baseball catcher. In 1947 he tried out for a Chicago Cubs farm team and failed. He was 23 when he had his first pro fight. Most champions start much younger.

Marciano made up for the late start with work. He ran at least five miles a day, often uphill. In training camp he was in bed by 9:30. His brother Peter said Rocky lived like a monk.

Jersey Joe Walcott was the heavyweight champion on September 23, 1952, when the two met in Philadelphia. Walcott dropped Marciano with a left hook in the first round. Marciano had never been knocked down as a pro. Cuts above his eyes blurred his vision for several rounds, and by the 12th Walcott looked firmly ahead on the scorecards.

Marciano knew a knockout was his only way to win. In the 13th round he landed an overhand right. Walcott went down and stayed down. Marciano retired in 1956 with a record of 49 wins, 43 by knockout, and no losses.

You will not win much by quitting halfway. Marciano never left a fight unfinished. Your bladder deserves the same standard, and today’s letter shows you how to check it.

THE TEST THAT CHECKS THE WRONG THING

You probably get a PSA blood test every year or two. The doctor reads the number and tells you the prostate looks fine. You leave the office feeling cleared, even though you still get up at night to pee.

I want you to know what that test leaves out. PSA measures a protein from your prostate in your blood. Those levels can go up when the prostate grows, so the number may hint at size. A PSA result still does not show how well your bladder empties. In other words, your doctor checks your PSA, but not what stays in your bladder after you go.

You might think the weak stream and the trips at night are just age. Some of that may be true. For many men, though, the real problem is urine that never leaves.

IT’S NOT AGE. IT’S WHAT STAYS BEHIND.

You may hear doctors call this chronic urinary retention. Three facts make it worth a closer look.

01 / It can hide for years

NIH experts say chronic retention may cause few or no symptoms, which makes it hard to catch. Signs can include a slow stream or trouble getting started. Some men feel they need to go again right after they finish.

02 / It gets common after 70

You are more likely to face this as you age. Cleveland Clinic says about 10 percent of men over 70 will develop urinary retention, and up to 30 percent of men over 80. Men with an enlarged prostate are the most likely to get it, because the gland presses on the tube that carries urine out.

03 / The leftover can do damage

Cleveland Clinic warns that urine sitting in the bladder is a breeding ground for bacteria. Left alone, retention can lead to infections, bladder stones, or an overstretched bladder. In some cases the trouble can reach the kidneys.

You blamed your age for the night trips.
Your bladder was never finishing the job.

ASK FOR THE SCAN

You do not need a new doctor. At your next visit, say one sentence: “Can you check how much urine I leave behind after I go?” Doctors call this a post-void residual test.

You pee first, then a nurse slides a small ultrasound probe over your lower belly. The Urology Care Foundation says the scan does not hurt and uses no radiation. Most men go home right after. Cleveland Clinic lists about 50 to 100 mL left over as a normal result for adults. Your doctor will tell you what number is too high for you.

You can also start one free habit tonight. Harvard Health suggests you double void: after you urinate, try again right away. Mayo Clinic adds that you should stop drinking an hour or two before bed.

I would call a doctor right away if you ever cannot pee at all. Cleveland Clinic calls sudden retention an emergency.

One last thing. You finish what you start everywhere else in your life. Make sure your bladder does too.

Earned it: 3 things you want

You fought hard for every round. Reward the finish.

01 / Watch · Rolex Air-King

Rolex released the Air-King in 1945 to honor the Royal Air Force pilots of the Battle of Britain. The current model has a 40mm steel case with a bold black dial made for pilots. Its US list price is about $7,450. Wear it every day, since Rolex built it to work.

02 / Drink · The French 75

The French 75 takes its name from a French field gun used in World War I. Bartenders build the classic version from gin, lemon juice, and a little sugar, topped with champagne. Some New Orleans bars make it with cognac instead. One glass is a celebration, so stop at one.

03 / Trip · A week on the Amalfi Coast

Italy’s Amalfi Coast clings to cliffs on the southern side of the Sorrento Peninsula. Positano, Amalfi, and Ravello are its best-known towns. UNESCO lists the coast as a World Heritage site, with terraced vineyards and citrus groves on the slopes. You climb a lot of stairs there, so your legs earn every dinner.

P.S. Before you buy a prostate pill, fix the free stuff first. Ask for the bladder scan, try double voiding, and stop drinking an hour or two before bed. A capsule cannot tell you how much urine you leave behind, but a simple scan can.
