Cut From Steel

HE SURVIVED THE ICE. HE WOULD NOT SURVIVE A CHECK-UP.

Ernest Shackleton sailed south in 1914 to cross Antarctica on foot. His ship never reached the continent. Pack ice trapped the Endurance in January 1915, and the ice crushed her that November. Shackleton and twenty-seven men then camped on drifting floes for months.

Shackleton's party reached Elephant Island in April 1916. He then took five men and one open boat into the Southern Ocean. That boat covered roughly 800 miles in fifteen days. He landed on South Georgia, crossed its unmapped mountains in about thirty-six hours, and walked into a whaling station. Every one of his twenty-eight men came home alive.

So Shackleton could survive almost anything. He could not face one ordinary thing.

Shackleton would not let a doctor examine him. His wife pushed him to see a heart specialist before an earlier expedition. A shipmate said he examined the specialist instead. In Rio de Janeiro in 1921 he suffered a heart attack, and he refused to let his own ship's surgeon look at him. Dr Macklin ordered rest. Shackleton carried on.

He died on 5 January 1922, hours after reaching South Georgia, aged forty-seven. Macklin later found extensive disease in the arteries of his heart. He beat the Southern Ocean. The examination he refused beat him. You should sit with that one.

 
 

THE NIGHT TRIP YOU ALREADY EXPLAINED AWAY

You get up twice a night now. Some nights it is three times. You worked out the reason years ago, and the reason was your prostate. Every man your age says the same thing, so it has to be true.

IT IS NOT ALWAYS THE GLAND

You wake up because your bladder is full. That is the whole mechanism, start to finish. So the real question is not only what squeezes the bladder… it is why the bladder keeps filling at two in the morning. Doctors call the overnight version nocturnal polyuria, which means the body makes too much urine while you sleep. Harvard Health notes that the hormone helping you hold fluid overnight drops naturally with age. Three ordinary things can make that worse.

01 / What You Drank After Six

You had two beers with the game and a coffee after dinner. Alcohol and caffeine both push your kidneys to make more urine. The Urology Care Foundation suggests limiting fluids two to four hours before bed. Check the pill bottle as well. Cleveland Clinic advises taking a water pill in the morning, or at least six hours before you sleep.

02 / The Fluid In Your Legs

You sit at a desk or behind a wheel most of the day. Fluid settles in your ankles while you are upright. You lie down at eleven, that fluid moves back into circulation, and your kidneys spend the night processing it. Harvard Health links daytime fluid retention and varicose veins to exactly this pattern. Raising your legs in the evening may shift some of that load before bed.

03 / How You Breathe When You Sleep

You snore, and your wife has mentioned the pauses. Untreated sleep apnoea is linked to more urine production overnight. Research suggests CPAP treatment can cut nocturia episodes by one or more per night. High blood sugar belongs on the list too, because your kidneys pull water out to dump the excess.

TWO NIGHTS AND ONE SHEET OF PAPER

You can turn this into something a doctor can use in two nights. Keep a bladder diary. Write down every drink, what it was and roughly how much. Note every trip to the bathroom and the time it happened. The NIDDK publishes a free daily bladder diary that fits on one page.

You then walk in with evidence instead of a vague complaint. A doctor can see at a glance whether you are making too much urine overnight or emptying too little on each trip. Those two problems have very different answers.

I am not telling you to rule your prostate out. A prostate problem stays a real possibility, and that exam still belongs on your list. Mayo Clinic says blood in your urine, pain when you pass it, or a sudden change should be seen promptly rather than tracked.

One last thing. You are not self-diagnosing here. Two nights of notes turns a complaint into a case. Shackleton had all the courage in the world. He still skipped the appointment.

You do not need to be young for this one. The FAA requires at least 40 hours of flight time for a private pilot certificate, though it notes the American average runs closer to 75. AOPA puts the whole bill somewhere between $6,000 and $20,000 or more. Start with a single discovery flight at your local field. An hour in the left seat tells you whether the rest is worth it.

 

P.S. Saw palmetto sits in nearly every prostate supplement on the shelf. NCCIH reports that a large trial and a Cochrane review both found it no better than placebo for these symptoms. So fix the free stuff first. Move your last drink earlier, put your legs up in the evening, and keep the two-night diary. Then have the prostate conversation holding something real.
